Myoclonus is a clinical sign that is characterized by brief, shock-like, involuntary movements caused by muscular contractions or inhibitions [ 1 ]. Muscular contractions produce positive myoclonus, whereas muscular inhibitions produce negative myoclonus (ie, asterixis).

Unverricht-Lundborg disease, Lafora disease) (see these terms) and other secondary forms of dystonia. Dystonia is caused by incorrect signals from the brain to the affected body part but the causes for this are not yet fully understood. However, many cases of myoclonus dystonia are inherited, caused by mutations in a specific gene. The fault is found on the DYT11 gene (or SGCE gene). In Myoclonus dystonia, the myoclonic jerks typically involve the muscles of the neck, trunk, and the hands. Involvement of leg muscles is rarely seen.

Although rare, myoclonus may be caused by an injury to the peripheral nerves (the nerves outside the central nervous system that connect to sensory organs and muscles, and relay information from/to the CNS). Because only the paternal copy of the SGCE gene is active, myoclonus-dystonia occurs when mutations affect the paternal copy of the SGCE gene. Mutations in the maternal copy of the gene typically do not cause any health problems. Rarely, individuals who inherit an SGCE gene mutation from their mothers will develop features of myoclonus-dystonia. It is unclear why a gene that is supposed to be turned off is active in these rare cases.

Physiologic myoclonus is often normal in people, for example myoclonic jerks during sleep transitions. Epileptic myoclonus referred to myoclonus in the setting of epilepsy.

Palatal myoclonus. Causes contractions near one or both sides of the roof of the mouth (soft palate). It can also affect the face, tongue, throat or diaphragm. MD can be distinguished from early-onset primary torsion dystonia, which also can cause myoclonic jerks (3, 5, 6). A 3-bp deletion in the DYT1 gene causes the majority of cases of early-onset primary torsion dystonia , and linkage to the DYT1 locus has been excluded in several families with MD (8, 9), including the one under study (data not shown).
